As we age, with time, and gravity skin on our face and throughout  the rest of our body tends to thin and stretch out. We end up with with excess skin pretty much all over the place. Specifically, for this topic, in the upper and the lower eyelids. So, there are several different procedures that can be performed in order to improve the appearance of one's eyes. Many patients often walk into my office and say, "doctor I always look tired. People are always asking me, you know, if I'm tired. If I got a good night's sleep, why I look so tired. I don't want to look tired anymore." Usually, this is related to the appearance of the upper and lower eyelids. Bags under the eyes. Excess skin under the eyes. And excess skin in the upper eyelids above the eyes lead to the appearance of first of all again and second of all stress and being tired. So, in order to improve the appearance of the eyelids we  do several things. First, we get rid of excess skin. Second, we tighten up the muscle. And, third, we remove excess fatty tissue that surrounds the eyeball itself. So, by combining the removal of skin, the tightening of tissue like muscle and tendons in the vicinity of the eye and by removing any excess fatty tissue we're able to improve the appearance and contour of both the upper and lower eyelid. In addition, there is often a crease or a groove underneath the lower eyelid which is called the tear trough deformity. This is an attachment of the skin to the underlying  bone. And, this gives the appearance of a deep dark line. This can either be addressed surgically or it can be addressed in the office by injecting a specific filler into this area. And, that filler will also improve the contour of the lower eyelid. So, when we're talking about lower eyelid surgery, from a plastic surgery perspective, or upper eyelid surgery. These are the surgical options.
